Effective March 15, 2021, FMLASource will begin administering Family and Medical Leaves for ABB Optical Group. This new process will continue to ensure that your leave decisions are made promptly, properly, and administered to protect your benefits and ensure that qualified claims are filed. Going forward, all new FMLA requests must be made directly to FMLASource. If you are on existing intermittent FMLA, you will need the submit a new claim through FMLASource to maintain accurate reporting.
Your information will be verified by a Benefits Specialist who will initiate the FMLA and/or STD leave process and answer any questions you may have.
You will be notified of the status of your FMLA leave once it has been processed.
You will continue to use your normal call out procedures notifying Supervisor/Manager.
You will then be required to call FMLASource of your absence.

Submit Your Leave | 15 Minutes
Submit your FMLA leave information to FMLASource via online or phone.
FMLASource handles the FMLA claim and completes the intake questions.
Receiving Your Leave Request Packet
Work with your health care provider to ensure medical documentation is completed and returned to FMLASource within 15 days.
FMLASource will fax the documents to your physician (if authorized) OR you provide the documents to your health care provider.
Review request packet and all attachments for important company information.
FMLASource Reviews Documents
FMLASource receives the completed documents and reviews them in order to issue a decision.
FMLASource may reach out to you or your health care provider (if authorized) for any incomplete information.
FMLASource Issues Decision | 5 Business Days*
You are then notified of the decision by email and/or postal mail and provided with the next steps, if necessary.
